Family support specialist and case management
Once you have gone through the intake process and accepted into our program, you will be assigned a personal one on one FSS to work with you in achieving your goals and to assist you and your family in preparing for permanent housing. For more information, schedule a free virtual pre-orientation.
Case management connect our clients with health, social service, and other treatment and support services, assuraning that our families have access to continuity of care.
Our case managers advocate on behalf of our clients and are responsible for empowering our clients to their fullest potential. Case management activities include initial and ongoing assessments of the client's needs, developing a comprehensive and individualized service plan with our program Intake Specialist; connecting clients to needed services; and monitoring the ISP to access its efficacy. Case management is a vital support service that assures a responsive and client driven system.

Workshops
The vision of transitions is to transition families from living in the motel community into permanent housing. In order to do this, we must EMPOWER them with the necessary
tools to make such transition. We offer weekly workshops for families participating in our program. You can join our workshops and not participate in our program, however, Workshops are not an option for participating clients.
